Atopic dermatitis, a chronic, itchy, inflammatory skin disease,
has caught the attention of the medical community in recent
years due to the disease’s significant increase in prevalence
over the last decades, particularly in the developed nations. As
a result, a substantial amount of investigations, in both clinical
and laboratory aspects, have been conducted in search for
answers of reasons underlying such increase of prevalence to
occur and for optimal therapeutic options. While many factors
have been viewed as contributors to the development of
the disease, some seem to be on the opposite spectrum of the
others, no single root cause has been identified. On one hand,
defect in certain skin barrier protein, like filaggrin, has been
found in some patients affected by atopic dermatitis, pointing
to external factors that penetrate the internal weakness of the
skin to cause skin inflammation. On the other hand, explicit
altering of the skin immune milieu by artificially expressing a
prominent Th2 cytokine interleukin-4 in the basal epidermis
results in the skin barrier-intact interleukin-4-transgenic mice
with a disease phenotype that fulfills the clinical diagnostic
criteria of human atopic dermatitis, pointing to triggering
internal factors of immune dysregulation.
In Atopic Dermatitis: Inside Out or Outside In? we attemptto analyze this burdensome disease from a distinct perspective.
The clinical and laboratory evidence in support of
internally initiated factors and those in support of externally
triggered causes are delineated separately. We aim to develop
better understanding of the different components that may
influence the initiation, progression, and sustainment of
this inflammatory disorder. The ultimate goal of this book is
to help clinicians who care for atopic dermatitis to develop a
therapeutic strategy consistent with the knowledge resulted
from the most updated evidence.
