March 8, 2020 was the first time I saw COVID-19. A woman over age 60 with diabetes who was connected to numerous lines and a respirator, lay still in an ICU room. I peered at her through the glass, scared of contracting this novel corona virus. She unfortunately succumbed to her illness as did so many that we cared for during that first surge. Unfortunately, diabetes was one of the pre-existing conditions that was associated with increased mortality from COVID-19. Thankfully, with subsequent surges we have seen fewer deaths.
COVID itself can cause new-onset diabetes in patients some of whom had no predictable risk factors. It can also worsen hyperglycemia in patients with prior diagnosis of diabetes, which can be a dilemma for the patient as well as inpatient and outpatient providers.
In order to cover all aspects of the relationship between COVID and diabetes, I enlisted the help of colleagues in both the United States and Europe who had experience in treating patients with diabetes. A multi-disciplinary approach was needed to manage these complex patients, thus the authors represent a variety of disciplines including pharmacology, endocrinology, nephrology, infectious disease, podiatry, vascular surgery, pulmonary/critical care, and general medicine. In addition, the authors represent different positions in medical care: medical students, residents, fellows, physicians, nurses, nurse practitioners, and pharmacists.
In the process of editing this book, I was fortunate to work with a dynamic group of people who were eager to participate. One of the biggest challenges is that with each new variant, there has been changes in medications as well as vaccines. The FDA no longer recommends the use of the first vaccines as well as some of the treatments that gained emergency use approval in 2020 or 2021.
