Endometrial cancer (EC) is the fourth cause of cancer of women in the United States and the sixth in the world.The knowledge of pathogenesis and molecular characteristics of EC is constantly growing: the dualistic classification (EC types 1 and 2) proposed by Bokhman more than thirty years ago and confirmed by the pivotal studies of Kurman et al. appears inadequate today, even in light of numerous molecular studies.
The standard surgical approach for EC is represented by hysterectomy and bilateral salpingo-oophorectomy with or without lymphadenectomy. In recent years sentinel lymph node mapping has emerged as an attractive approach for the selecting patients who may benefit more from pelvic or lombo-aortic lymph nodal dissection. Its feasibility in early-stage EC has been confirmed by several studies, but it is not clear which is the best
mapping technique and injection site of tracer. Thus, sentinel lymph node mapping is not routinely performed in clinical practice.The best choice of adjuvant therapy remains an open question: until few years ago, radiotherapy (RT), which includes the external radiotherapy (RT), brachytherapy (BT) or the combination of these, was systematically employed in women with intermediate-high risk
and low risk EC, according to the FIGO 2009 classificationCurrently, innovative agents inhibiting critical molecular pathways, such as epidermal growth factor receptor (EGFR), vascular endothelial growth factor (VEGF), mammalian target of rapamycin (mTOR), are interesting experimental opportunities for the
treatment of patients with advanced EC.The limited available data on elderly patients with EC suggest that age alone should not be a sole critical factor in deciding medical or surgical treatment. Older patients should be always assessed for their fitness before receiving any type of surgical and medical treatment
