In this era of medical specialisation, we are increasingly becoming skilled at our jobs and
deskilled in managing areas out with our specialist expertise. The Royal College of
Obstetricians and Gynaecologist’s High Quality Women’s Health Care report of 2011 proposed
a number of changes needed in the education and training of doctors in the
specialty of obstetrics and gynaecology. As a result, the Advanced Training in Obstetrics
and Gynaecology document was published in August 2019, which addresses the purpose,
learning outcomes and content of learning, in addition to the core curriculum requirement
for the Certificate of Completion of Training (CCT). Neither the core curriculum
nor the obstetric Advanced Training Skills Modules (ATSMs – Advanced labour ward;
Fetal medicine; High-risk pregnancy; Labour ward lead; Obstetric medicine) include
management of the complex gynaecological problems encountered commonly during
pregnancy or in the post-natal period. And yet, obstetricians who have completed the
above-listed obstetric ATSMs would be expected to competently manage women with
complex gynaecological problems. This book provides the knowledge base to improve
the quality of care provided to pregnant and post-natal women suffering from complex
gynaecological problems. It will help to manage patient expectations and thereby
improve patient satisfaction. It will also help decrease complaints, litigation and claims
by providing appropriate management and protocols. And last but not least, it will be
hugely beneficial to obstetricians working within and outside the UK who are managing
common and less common gynaecological problems that occur in pregnancy.
