Medicine referred to as focused empiricism, military surgeons
managed a once-in-a-generation burden of vascular injury
within a new and evolving global trauma system.1 Newly
designed tourniquets, balanced transfusion of blood products,
damage control surgery, including the use of temporary
vascular shunts, and selective venous and tibial artery
repair were among the approaches that became standard
during the wars. For the first time a closed, negative pressure
wound dressing technology was used to control soft
tissue injuries associated with vascular trauma and endovascular
devices were applied to select injury patterns in
